Security System Site Information

In order to set up your alarm monitoring account we need to collect some basic information. Before you begin, you must have your alarm permit number from the City or County that has jurisdiction. If you don’t have this, obtain your permit from your jurisdiction’s website, and then return to complete this form. Please fill out all the fields below. Thank you.

This is the location where the alarm system is installed.

Site Phone

This should be the landline number for the site. If there is no landline, leave this field blank.

Alarm Permit Number

In order for your alarm system to be monitored, you must provide an alarm permit number from your City or County jurisdiction.

Permit Jurisdiction

This is the City or County that issued your alarm permit.

Codeword #1(Required)

If there is a false alarm, the monitoring company will ask you for your codeword, to verify you are the owner. This should be something everyone who uses the system will know, and be easy to remember.

Codeword #2

This can be a second codeword and is optional.

Master User Name (User #1)(Required)

This master user can add and delete other user codes and perform basic programming, such as entering date and time.

This should be a 4 digit number that can be used to disarm the alarm, or add and delete other users.
